A semen analysis is the cornerstone of male fertility assessment and the most important laboratory test used to evaluate a man’s reproductive potential.

The test examines both the macroscopic and microscopic characteristics of the semen, following the latest World Health Organization (WHO 2021) guidelines. It evaluates key parameters including semen volume, sperm concentration (count), motility (movement), and morphology (shape). A comprehensive semen analysis may also include additional specialized assessments, such as sperm viability and the presence of inflammatory cells (white blood cells), providing a more detailed evaluation of semen quality.

Sample Collection

The semen sample can be collected either:

  • At our fertility centre, in a dedicated private collection room designed to ensure comfort, privacy, and discretion.
  • At home, provided the sample is delivered to the laboratory within 30–45 minutes of collection and kept at a stable temperature of 20–25°C during transport.

Why Is a Semen Analysis Important?

A semen analysis provides valuable information about male reproductive health and is often the first step in investigating fertility. The results help guide further evaluation and, when necessary, assist in selecting the most appropriate treatment options.

While a normal semen analysis is reassuring, it does not guarantee natural conception, as successful pregnancy depends on multiple factors involving both partners.

What Should I Know Before Providing My Semen Sample?

To ensure the most accurate and reliable results, it is important to follow the instructions below before providing your semen sample.

A. Period of Abstinence

It is a common misconception that the longer a man abstains from ejaculation, the better the semen analysis will be. In fact, scientific evidence shows that this is not the case.

An abstinence period of 2–5 days before sample collection is considered ideal for semen analysis. Regular ejaculation has been shown to support the natural renewal of sperm quality, helping to maintain healthier sperm cells.

B. Medical History

Sperm production (spermatogenesis) is a continuous biological process that takes approximately three months. As a result, a semen analysis reflects a man’s overall health and lifestyle during the preceding months rather than just the days before the test.

Factors such as illness, fever, certain medications, or periods of significant physical or emotional stress may temporarily affect semen quality. If any of these have occurred recently, your doctor may recommend repeating the test after approximately three months, allowing a new cycle of sperm production to be completed.

For this reason, it is important to complete your medical history questionnaire as accurately and thoroughly as possible.

C. Complete Sample Collection

Not all portions of an ejaculate contain the same concentration of sperm or other important components. Consequently, the loss of any part of the sample during collection may influence the accuracy of the results, including sperm concentration and other semen characteristics.

It is therefore important to collect the entire ejaculate. If any portion of the sample is accidentally lost, please inform the andrology or laboratory staff, as this information is essential for the correct interpretation of your results.

What Does an Abnormal Semen Analysis Mean?

An abnormal semen analysis does not necessarily indicate infertility. In many cases, the findings are reversible and may simply require further investigation or follow-up. Small lifestyle changes can have a positive impact on sperm production (spermatogenesis) and may improve abnormal semen parameters, such as sperm concentration, motility, and morphology.

Should I Repeat My Semen Analysis?

Yes. A repeat semen analysis is often recommended to confirm the results, as sperm production (spermatogenesis) can be influenced by factors such as stress, infections, or recent illness. Repeating the test helps provide a more accurate and reliable picture of male fertility.

When Can I Repeat My Semen Analysis?

A repeat semen analysis is usually recommended after 2 – 3 months, which corresponds to one complete cycle of sperm production (spermatogenesis). Your doctor will determine the most appropriate timing based on your individual circumstances.

Does Varicocele Affect Sperm Quality?

Yes. Varicocele can affect sperm production and semen quality; however, this is not the case for every man. Assessment is based on a semen analysis and a clinical examination. Before undergoing varicocele surgery, sperm cryopreservation is recommended.

Can Medications Affect Male Fertility?

Yes. Certain commonly used medications may temporarily affect sperm production (spermatogenesis). Always inform your doctor or the laboratory about any medications you are taking.

Do Medications for Erectile Dysfunction Affect Sperm Quality?

No. Medications used to treat erectile dysfunction do not have a negative effect on sperm quality.

Sperm DNA Fragmentation Testing (DFI)

The DNA Fragmentation Index (DFI) measures the percentage of sperm carrying damage to their genetic material (DNA). A high DFI is one of the causes of male infertility and is not reflected in the conventional parameters of a standard semen analysis.

A sperm cell with damaged DNA may have normal morphology, progressive motility, and still be capable of fertilizing an egg. As a result, these sperm may appear normal on a routine semen analysis. However, the damaged DNA they carry may reduce the chances of embryo implantation and increase the risk of miscarriage. The test is performed using specialized molecular techniques, such as TUNEL or SCSA, and the results help guide decisions regarding the most appropriate treatment or the selection of assisted reproductive techniques.

Why Is This Test Important?

A high level of sperm DNA fragmentation can negatively affect reproductive outcomes, regardless of the method of fertilization (natural conception, IUI, IVF, or ICSI), by:

  • Reducing the chances of natural conception
  • Impaired or arrested embryo development
  • Lower implantation rates and an increased risk of miscarriage

Embryos resulting from sperm with fragmented DNA have a lower likelihood of leading to a full-term pregnancy, as the accumulation of DNA damage reduces the chances of achieving a normal and healthy pregnancy.

Men with:

  • An abnormal semen analysis.
  • Advanced paternal age.
  • Varicocele.
  • Long-term exposure to harmful or toxic substances.

Couples with:

  • Unexplained infertility.
  • Recurrent miscarriages.

When Should Sperm DNA Fragmentation Testing Be Performed?

Information about sperm DNA integrity helps the clinician diagnose, evaluate, and manage male infertility. The earlier the correct diagnosis is established, the more time, money, and effort can be saved for the patient. Although sperm DNA fragmentation testing can be performed at any stage of the male infertility investigation, it is advisable to include it early in the diagnostic process.

What Are the Main Factors Associated with Increased Sperm DNA Fragmentation?

Increased sperm DNA fragmentation may be associated with:

  • Exposure to chemicals, toxic substances, radiation, or high temperatures.
  • Air pollution.
  • The use of certain medications, anabolic steroids, and recreational drugs.
  • Varicocele, testicular cancer, inflammation, or high fever.
  • Increased testicular temperature.
  • Smoking and poor nutrition.
  • Advanced paternal age.
  • Infections, inflammation, and high fever.

In addition, sperm DNA fragmentation testing complements the comprehensive evaluation of male fertility during the investigation of infertility. It is particularly useful:

  • When monitoring the effectiveness of antioxidant supplementation.
  • Following surgical repair of a varicocele.
  • When assessing sperm quality in men who have undergone intensive cytotoxic treatments.
 What Is Sperm DNA Fragmentation and What Causes It?

Sperm DNA fragmentation refers to damage that accumulates in the genetic material (DNA) of sperm during their maturation, storage in the testes and epididymis, and ejaculation. This damage, which is usually the result of increased oxidative stress, can be caused by a variety of factors, including lifestyle habits, infections, varicocele, and many others.

 When Is Sperm DNA Fragmentation Testing Recommended and Why Is It Important?

Sperm DNA fragmentation testing is an important assessment that complements a standard semen analysis. While a semen analysis evaluates the macroscopic and microscopic characteristics of semen, DNA fragmentation testing provides information about the genetic integrity of the sperm.

The test is recommended in cases of unsuccessful pregnancy attempts, recurrent miscarriage, and unexplained infertility. It is also recommended following assisted reproductive treatments that have resulted in an unexpectedly low number of good-quality embryos, implantation failure, or miscarriage.

Is There Treatment for High Sperm DNA Fragmentation?

Yes, in many cases. Treatment may include lifestyle changes, such as quitting smoking, reducing alcohol consumption, and limiting exposure to toxic substances.

Management may also include the treatment of infections, antioxidant supplementation, and/or medical intervention, such as surgical treatment for varicocele.

Oxidation-Reduction Potential (ORP) Testing

Oxidation-Reduction Potential (ORP) testing evaluates the balance between oxidants (reactive oxygen species/free radicals) and antioxidants in semen.

While small amounts of reactive oxygen species are essential for normal sperm function, excessive levels can negatively affect sperm quality by damaging sperm DNA, lipids, and proteins. This can impair sperm function and ultimately lead to sperm cell death.

Testing for oxidative stress in semen is also recommended in the latest guidelines of the European Society of Human Reproduction and Embryology (ESHRE) for the evaluation of male infertility.

Who Should Have This Test?

ORP testing is recommended for:

  • Men with an abnormal semen analysis.
  • Men over the age of 40 who are planning to have children.
  • Men with a history of varicocele.
  • Men with infections of the urogenital tract.
  • Men with lifestyle factors associated with frequent exposure to harmful influences, such as poor nutrition, insufficient or excessive physical exercise, chronic stress, radiation, smoking, or toxins.
  • Couples with unexplained infertility.
  • Couples with recurrent miscarriage.
  • Couples with previous unsuccessful IVF treatment.
  • Couples with low fertilization rates or poor-quality embryos in IVF cycles.
What Factors Affect ROS Production and Oxidation-Reduction Balance?

The production of reactive oxygen species (ROS), and consequently the oxidation-reduction balance, can be influenced by:

  • Environmental factors, including chemical exposure, air pollution, radiation, and high temperatures.
  • Lifestyle factors, such as lack of sleep, stress, smoking, alcohol consumption, and obesity.
  • Advanced age.
  • Inflammation, infections, and various systemic diseases.
Why Is This Test Important?

It is estimated that approximately 40% of men with a normal semen analysis experience fertility problems. A significant proportion of these men are believed to have increased sperm DNA fragmentation and elevated levels of oxidative stress.
